What To Expect
Once you complete the form, your information with be sent to the Counseling Center. The case manager will reach out to you at the phone number you provided. Please ensure that they will be able to leave you a voice message if you are not able to answer. They will attempt to contact you twice regarding your request. If you have not heard from the case manager within 3 business days, please reach out to the Counseling Center directly at 205-348-3863. Please ask to speak to the case manager about your Project Thrive referral. The case manager will help you schedule your initial appointment.
